A convienience fee equal to the fee PayPal charges CSQ will be added ($.30 + 2.9% of total). You are no longer required to be a PayPal member in order to use their sevices.

If you do not wish to register online, please print the form on the next page and mail it to the address listed below.

A late fee of $20.00 is due on all registrations postmarked after January 16, 2013. Refund of registration fee, less $10 for handling, will be made if cancellation is in the hands of the registrar in writing by January 10, 2013 at midnight.
